Terms of Use for Contacting the EAP Electronically*

The participant must read all of the terms below to fill out the online contact form in its entirety.
1. Kepro’s Employee Assistance Program follows all federal regulations regarding confidentiality and privacy. We do not release any information about you to anyone without your written permission unless you are in danger of harming yourself or someone else, or if we suspect child or elder abuse, or as required by law.
2. Participant maintains that they are safe, and they are not in any immediate danger (i.e., experiencing any thoughts to harming themselves or others, active violence in their relationship, or experiencing withdrawal symptoms from drugs and/or alcohol). If you are in immediate danger, please call your dedicated toll-free phone number or Kepro EAP at 1-800-607-1522 or proceed to the nearest emergency room and/or call 911.
3. Participant understands that they will receive a call or email from an EAP Consultant, within one business day (excluding weekends and holidays), to assess the participant’s needs and assist in connecting with local referrals. Please allow for up to 15 minutes of uninterrupted time for this intake and referral.
4. An EAP Consultant will make two attempts to reach the participant. Participant agrees that they have a least one phone number or email address where a detailed, confidential, message can be left. If we are not able to reach you by the second attempt, we will not make any further attempts until we hear back from you.
5. By clicking ‘Accept’, participant is agreeing that they have read and understood these terms as well as providing an ‘electronic signature’ allowing the EAP to contact them to initiate services.

Your employer has partnered with Kepro to provide the Employee Assistance Program (EAP). EAP consultants are employees of Kepro.
