Terms of Use for Contacting the EAP Electronically*

1. The participant must read all of the terms below to fill out the online contact form in its entirety.
2. Participant maintains that they are safe and they are not in any immediate danger (i.e. experiencing any thought to harm self or others, active domestic violence relationship, or experiencing withdrawal from drugs and/or alcohol). If the participant is in immediate danger they should call their dedicated toll-free EAP phone number or proceed to the nearest emergency room and/or call 911.
3. Participant understands that they will receive a call back from a Master’s-level CareLink Consultant, within one business day (excluding weekends and holidays), to assess the participants needs and assist in connecting with local referral resources. Please allow for up to 15 minutes of uninterrupted time for this intake and referral.
4. A CareLink Consultant will make two attempts to reach the participant. Participant agrees that they have a least one phone number where a detailed, confidential, message can be left. If we are not able to reach you by the second attempt we will not make any further attempts until we hear back from you.
5. By clicking 'Accept', participant is agreeing that they have read and understood these terms as well as providing an 'electronic signature' allowing the CareLink program to contact them to initiate services.
